If it’s your first time calling us and you are unsure which therapist you would like to work with, please DIAL OPTION 1 and leave a voicemail for our Client Coordinators. They will return your call within 24 hours, Monday - Friday and speak with you about scheduling an Intake Appointment.

SCHEDULE DIRECTLY WITH A THERAPIST
We welcome you to book your session directly with a clinician. You can find each clinician's contact information below in the Therapist Directory.
After scheduling your first appointment, you will be directed as to how to complete new client forms. You may choose to:
Receive a secure email with a link to the paperwork OR
Arrive 30 minutes prior to your first scheduled appointment to complete the paperwork made available to you in our waiting room.
New client paperwork should be completed separately by each adult and/or youth attending therapy. If you have any questions about the paperwork, please call the main office at 636-379-1779 and speak to our front office staff.
WHEN YOU ARRIVE FOR YOUR APPOINTMENT, please have a seat and make yourself comfortable in the waiting area. Your therapist will greet you at the time of your scheduled appointment.
NEW CLIENT PAPERWORK For new clients who have scheduled an appointment, our Intake Coordinator will email you a link to complete your forms online.
For your reference and convenience, the following documents are made available to you: Client's Bill of Rights This is a statement that outlines a client's rights to the therapy process in addition to rights outlined in registration.
Notice of Privacy Practices No signature is required on this form. Please read it over so you are aware of your privacy protections. You will be asked to initial the Disclosure Statement and Consent to Treatment form indicating that you have read it.
